CELEBREX® REFILL PROCESS

Your original consultation includes 3 refills over a period of 6 months. Remember that you may order 60, 100 or 200 pills per prescription. Each prescription is independent of the other. This allows you to order different quantities for each refill. To receive a refill please complete the following steps:

1. Read and agree to the Waiver of Liability.

2. Select the quantity of Celebrex® pills.

100 mg Celebrex® Pills

  60 - 100 mg doses $105 ($1.75/100 mg dose)
100 - 100 mg doses $170 ($1.70/100 mg dose)
200 - 100 mg doses $330 ($1.65/100 mg dose)

200 mg Celebrex® Pills

  60 - 200 mg doses $156 ($2.60/200 mg dose)
100 - 200 mg doses $255 ($2.55/200 mg dose)
200 - 200 mg doses $500 ($2.50/200 mg dose)

WAIVER OF LIABILITY

I hereby release www.onlinepills.com and all of their employees and contractors including physicians from all liability associated with my Celebrex® consultation and/or the use of Celebrex® as listed by Searle/Pfizer.  I understand that no physician, nurse or administrative personnel can guarantee that Celebrex®, even if prescribed, will provide the results I seek.  I understand that falsifying information in order to obtain a prescription medication is a violation of both state and federal laws.  I also understand that if I fail in anyway to furnish www.onlinepills.com with my complete and accurate medical history or become aware of any changes in the future which I have not notified www.onlinepills.com of then I cannot hold them responsible for any adverse effects I may suffer.

I am fully aware that it is my responsibility to have an annual physical exam, including any suggested laboratory test, to ensure that I have no medical condition, which might make Celebrex® inappropriate for me.  I also understand that this consultation is not a substitute for my need to visit a local physician for my annual exam.  I further agree to consult with my local physician and/or pharmacist to warrant that I am not taking any medication on the published list that would contraindicate the use of Celebrex®.  I further agree to notify all physicians who I am currently under or any physician who I will engage in the future, of my decision to use Celebrex® so they may advise to continue or discontinue the use of medication.

I am aware of the potential side effects and drug interactions of Celebrex® as listed by Searle,Pfizer @www.celebrex.com.   Although Celebrex® is generally well tolerated, there may be side effects including but not limited to indigestion, diarrhea, abdominal pain, etc.  I also realize that serious gastrointestinal toxicity such as bleeding, ulceration and perforation of the stomach, small intestine or large intestine can occur at anytime, with or without warning symptoms, in patients treated with non-steroidal anti-inflammatory drugs (NSAIDs).  Therefore, individuals should remain alert for ulceration and bleeding, even in the absence of previous GI symptoms.  I Finally, I understand a qualified licensed physician, who may or may not be licensed to practice medicine in my state, will evaluate the information I am providing.

If approved, I irrevocably www.onlinepills.com to be my agent and have my prescription and any refills filled by the Pharmacy of its choice and acknowledge that the Rx obtained for me is non-transferable. We are unable to accept returns or issue refunds for any orders due to the fact that this is a prescription medication.
